CONSENT AND AUTHORIZATION FOR BACKGROUND CHECK
Before you proceed, please carefully read the following information. Your informed consent is required to conduct a background check.
This document explains your rights and how Cross-Check Background Reporting Inc. ("Cross-Check," "we," "us," "our") collects, uses, and discloses your personal information for the purpose of conducting a background check. By checking the boxes below, you acknowledge that you have read, understood, and agree to these terms.
1. General Acknowledgement and Consent
By checking the box below, you, the undersigned, authorize Cross-Check to procure a background check report about you. This report may be prepared for an entity that has requested it (the "End-User," e.g., a potential employer) or for your own personal use ("self-check").
You understand that the background check may include, but is not limited to, verification of your identity, education, employment history, professional licenses, and searches of criminal records, credit history, and driving records, as permitted by law and as specifically authorized by you below.
You authorize Cross-Check to share the information you provide and the results of the background check with the End-User who has requested the report. If you are ordering a self-check, the report will be provided only to you.
2. Consent to Specific Types of Checks
Please provide your explicit consent for each type of check you authorize us to conduct. The End-User have specified which checks are required. For this application type of background check has been requested by the End-User.
3. Disclosure to Third Parties and International Data Transfers
To provide our services, we may need to share your personal information with third-party service providers and government entities. These third parties are contractually obligated to protect your information and use it only for the purposes for which it was shared.
Your information may be transferred to, and stored and processed in, countries other than your country of residence, including the United States, Canada, and member states of the European Economic Area. These countries may have data protection laws that are different from the laws of your country. We will take all reasonable steps to ensure that your data is treated securely and in accordance with our Privacy Policy and applicable laws.
You authorize Cross-Check to share the information you provide and the results of the background check with the End-User who has requested the report. If you are ordering a self-check, the report will be provided only to you.
4. Certification and Release
I certify that all the information I have provided is true, accurate, and complete. I understand that any false statements may disqualify me from consideration for employment or other opportunities.
I hereby release and discharge Cross-Check Background Reporting Inc., its agents, and its clients (the End-Users) from any and all liability, claims, or demands for damages, loss, or injury arising out of the collection, use, and disclosure of information in connection with this background check, to the fullest extent permitted by law.
